As the population ages, so does the need for compassionate support and services that enable seniors to remain independent. We offer home modifications for greater safety, affordable apartment communities with service coordination and volunteer opportunities to stay involved in the community.

A Model for Aging in Place

The Terraces on Tulane’s Aging with Options program is intended to improve residents’ ability to self-manage chronic health conditions, promote resident wellness, including social engagement, and support residents’ ability to age in place. Aging with Options is built around the presence of an onsite Service Coordinator, who plays an important role in assisting residents with social determinants of health needs.

Aging with Options works with Humana to help plan members address their health and wellness needs, and with Heal to provide onsite primary care to residents, who choose to seek services. The Aging with Options team consists of a Community Health Worker, Wellness Nurse, and Service Coordinator, who will provide one-on-one assistance and group programming to Terraces on Tulane residents with interrelated health and wellness needs. The program assists residents in a variety of areas of their lives, such as when they return from the ER or a hospital visit. Other services provided by the program include health education and coaching, among others.
